This site has two separate types of forms, and they are not treated the same way:
(a) The Appointment Booking Form (demonstration only)
Information entered here is used exclusively to demonstrate the booking workflow (status updates, automated email notifications). Please do not submit real medical information or sensitive personal data through this form.
(b) The “Request a Custom System Quote” Form (real business inquiry)
Unlike the booking form, this is a genuine channel for prospective clients to contact JuanDigital about commissioning custom software. If you submit this form, you are providing real contact information for a real business purpose, and we process it as follows:
- What we collect: Full name, company name, email address, mobile number, existing website (if provided), and project details.
- Why we collect it: To respond to your inquiry, prepare a quotation, and follow up regarding potential engagement of our services.
- Verification: We verify your email address via a one-time code before your inquiry is logged, to keep our records free of spam and invalid submissions.
- Where it’s processed: Your submission is stored in our secured database and may also pass through our internal workflow and notification tools (currently n8n and Google Sheets), used strictly to manage and respond to your inquiry.
- Retention: We retain quotation inquiries for up to 12 months, or until you request deletion, whichever is earlier.
- Your rights: Under the Philippine Data Privacy Act of 2012 (RA 10173), you have the right to be informed, to access, to object, to correction, to erasure or blocking, to data portability, to file a complaint, and to damages.
